Forms can be printed, filled in, and then brought to the District of Ucluelet Parks and Recreation office at 500 Matterson Drive or filled in and emailed to Barb Gudbranson at bgudbranson@ucluelet.ca or to our Special Events Coordinator at ukeedays@ucluelet.ca.
